You'll need to provide:
- your personal details
- two forms of verification so we can confirm your identity
- your pet's lifetime tag and microchip number
- proof of registration class if changing your pet's class.
If you're changing your legal name, you'll need to upload a copy of a legal document (eg, marriage licence, drivers' licence, passport, proof of age card or birth certificate).
