Request an immunisation statement

The record of all vaccines administered by the Yarra Ranges Council immunisation service is forwarded to the Australian Immunisation Register.  You can get your immunisation history statement straight away using either:

• your Medicare online account through myGov 
• Express Plus Medicare mobile app.

If you do not have access to either of the above or if you have any questions, please phone the Yarra Ranges Council immunisation service on 1300 368 333.

You can also call the Australian Immunisation Register on 1800 653 809 and ask them to send the immunisation history statement to you.

If your child has received vaccines in another country these records can be added to the Australian national register.  Please complete the immunisation record update request form and provide a copy of the vaccines administered.  If the records are not in English, you may need to have them translated prior to completing the application.
