Before you start

Complete this form to let us know there has been a change of pet ownership of a pet registered in Yarra Ranges Council.

You should only use this form to apply to change the ownership of a pet currently registered in Yarra Ranges Council.

The previous and current owner must both live in the Yarra Ranges Council area.

Do not use this form to transfer a pet previously registered in another council. To register a new pet, please visit Register your pet.

To complete this form, you will need to provide:

  • previous owner details
  • current owner details
  • pet registration reference or tag number
  • pet microchip number (if known)
  • a statutory declaration confirming ownership OR proof of a change of ownership with a microchip registry
  • proof of concession eligibility, if applicable.
