Support for seniors

We provide services to support eligible residents.These services are intended to assist older people to remain independent at home, while remaining connected to the community.

Services available include:

Food services - operating ‘contactless’ wherever possible

This service is effected by the Cornonavirus situation and will continue to operate but will be 'contactless' wherever possible
 

We cater for all dietary requirements including gluten free, vegetarian, diabetic and textured modified food.

All our meals meet the nutritional standards of the National Standard Guidelines. Meals consist of soup, a main, dessert and juice.

Meals can be delivered to your home or collected from Monday to Friday from one of our three kitchen locations in:

  • Upwey
  • Lilydale
  • Yarra Junction

You may be eligible if you meet Commonwealth Home Support eligibility criteria or are in receipt of an Aged Care Package.

Domestic assistance - Modified service

Modified service - will be restricted to those clients who are highly dependent on the service and have no alternative supports available

Domestic assistance may include general house cleaning, shopping, clothes washing and ironing. Eligible applicants must meet Commonwealth Home Support eligibility or be in receipt of an Aged Care Package.

Respite care - Temporarily suspended

Temporarily suspended

We offer respite care services including centre based respite and flexible respite care.

Eligible applicants must meet Commonwealth Home Support eligibility or be in receipt of an Aged Care Package.

Personal care - Modified service

Modified service will be restricted to those clients who are highly dependent on the service and have no alternative supports available

Services will be delivered to clients who wish to receive them, and who are assessed as being in high-need and who have no other suitable supports. 

Personal care includes assistance with activities of daily living for hygiene and grooming. Activities may include eating, bathing, toileting, dressing and grooming.

Eligible applicants must meet Commonwealth Home Support eligibility or be in receipt of an Aged Care Package.

Home maintenance - Modified service

Modified service will only take place where the client's immediate safety will be impacted if the service does not occur

Services include home and yard maintenance that can help you to remain living in your own home. Home maintenance services exist to address needs in safety, accessibility and health. These services are not to be used for purely aesthetic reasons. Th

Activities funded can include a range of maintenance or repair tasks such as:

  • Repair of flooring

  • Minor plumbing, electrical & carpentry repairs where safety is an issue

  • Working-at-height related repairs or cleaning such as gutters, roofs, windows, ceilings, smoke alarms

  • Secure access issues for personal safety

  • Accessible, low maintenance garden redesign

  • Yard maintenance such as pruning, yard clearance or lawn mowing where there are issues for safety and access. 

Eligible applicants must meet Commonwealth Home Support eligibility or be in receipt of an Aged Care Package.

Social support groups - temporarily suspended

This service is temporarily cancelled due to the Coronavirus situation.

Social support groups provide an opportunity to participate in social events and activities outside of your home.

Activities may take the form of:

  • group-based activities held in a facility/centre

  • group excursions held outside of a centre.

Services may include meals and associated transport and personal assistance (e.g. help with toileting) involved in attendance at the centre.

Eligible applicants must meet Commonwealth Home Support eligibility or be in receipt of an Aged Care Package.

Transport services - temporarily suspended

This service is temporarily cancelled due to the Caronavirus situation. 

Yarra Ranges Community Bus Transport Service can assist you to connect with the community. It is available for shopping trips, outings, transport to seniors clubs and medical appointments. You can be picked up and returned to your home.

The service operates in different areas across the Yarra Ranges from Monday to Friday.

You may be eligible if you meet Commonwealth Home Support eligibility criteria or are in receipt of an Aged Care Package.

Current shopping trips

Warburton Shopping - operates every Monday picking up at locations along or near the Warburton Highway travelling to Chirnside Park Shopping Centre.

Chirnside Park Shopping - operates every Tuesday picking up from Montrose, Lilydale and Chirnside Park travelling to Chirnside Park Shopping Centre.

Upwey Shopping - operates Tuesday on a fortnightly basis, picking up at Hills locations along the way such as, Upwey, Tecoma, Belgrave, Belgrave South, Selby, Menzies Creek and Narre Warren North travelling to Endeavour Hills Shopping Complex.

Monbulk Shopping - operates Thursday on a fortnightly basis, picking up at Monbulk and Wandin East travelling to Chirnside Park Shopping Centre.

 

 

How do you access these services?

You will be assessed to determine your eligibility and the types of services required.

If you are 65 and older (or 50 and older for those of Aboriginal and Torres Strait Islander background)

Contact My Aged Care on 1800 200 422 to find out if you are eligible for the Commonwealth Home Support Program.

If eligible, you will be referred to the Regional Assessment Service, who will arrange a home visit to discuss your needs and arrange support.

Should more intensive long term assistance be needed, the Aged Care Assessment Service (ACAS) will be engaged to explore a package of care (through another Provider) or residential aged care.

People aged 64 and younger (or 49 and younger for Aboriginal and Torres Strait Islander background)

You may be able to access the state funded Home and Community Care Program for Younger People.

Contact us on 1300 368 333 to find out if you are eligible. An Assessment Officer will visit to establish eligibility and develop a support plan with you.

This service is intended to provide short term, basic support until the individual is able to resume normal activities.  Long term support may be possible and is assessed on a case by case approach.
