Support for Older Residents

Yarra Ranges Council, through Aged and Disability Services Department, provides a range of support to eligible residents as part of the Home and Community Care (HACC) program.

The overall aim of the HACC program is to assist

  • frail older people
  • people with some form of disability, including dementia 
  • carers in their caring role

to maximise their independence and remain living at home, stay connected to their community and adapt to changing life circumstances.

To this end, Council provides a range of coordinated and integrated service approaches.
